Source of Hope LLC is a dynamic and growing organization dedicated to making a positive impact in our community. We are seeking a highly motivated and organized Bilingual Case Manager (English-Spanish) to join our team and help us achieve our mission. The Case Manager will be responsible for assessing, planning, coordinating, monitoring, and evaluating the services required to meet an individual's health and human services needs. The Case Manager will work with clients, their families, and providers to develop and implement individualized service plans, ensuring the client receives the necessary support and resources.

Key Responsibilities:

- Plan and follow program activities, including scheduling, logistics, and resource allocation.
- Collaborate with team to develop program goals, objectives, and strategies.
- Monitor program progress and make adjustments as necessary to ensure objectives are met.
- Develop and maintain relationships with families.
- Prepare and deliver program reports and updates to management.
- Assist in marketing and promoting programs to increase participation and support.
- Evaluate program outcomes and gather feedback for continuous improvement.

Qualifications:

- Bachelor's degree in a related field or equivalent work experience.
- Fluent in both English and Spanish.
- Proven experience in Case management or a related role.
- Strong organizational and project management skills.
- Excellent communication and interpersonal abilities.
