Job Description
Position Summary
Promotes healthy lifestyle in managing residents care experience through proper communication flow between the community's onsite Medical Clinic, therapies, ancillary services, and community interdisciplinary team. Develops partnerships with service providers and agencies for resident referrals. Educates residents on available services and monitoring provisions of services. Responsible for fostering a continuum of care for residents. Serves as a liaison between families, residents and healthcare professionals as needed.
Essential Duties
  • Conducts initial and periodic reviews of residents to determine ability for admittance, feasibility of continued residence within community housing as well as health care and health promotion needs
  • Monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory.
  • Provides limited case management needs for a resident when the general service community is not providing such service.
  • Educates residents on service availability, application procedures, resident rights, etc. both individually and as a group.
  • Refers residents to volunteer support group programs with service organizations in the community.
  • Assists management in identifying residents who need assistance and may help in coordinating that assistance.
  • Maintains individual resident files and documents all pertinent information.
  • Responds to resident emergency requests for assistance as necessary as part of the response team.
  • Other duties as assigned.

Qualifications
Basic Qualifications & Experience
  • Licensed Practical Nurse license, in good standing in state of residency.
  • Minimum 2 years working with senior adults, preferably in a Senior Care living community.
  • Able to effectively respond in crisis situations.
  • Must possess excellent customer service skills.
  • Able to read, write, and speak with English language

Working Conditions / Physical Requirements
  • Moderate physical activity required by handling objects up to 50 pounds occasionally and/or 20 pounds frequently.
  • Possible exposure to chemicals, blood borne pathogens and other infectious diseases.
  • Sits, stands, bends and moves frequently during working hours.
  • Is subject to frequent interruptions.
  • Interacts with residents, family members, visitors frequently
  • Frequently interacts with relevant stakeholders including team members, developers, consultants, brokers.
