Director of Intake – Home Health

We are currently working with a very progressive, well run and financially stable Home Healthcare organization that serves the Sarasota, FL area. They are committed to delivering high-quality patient care while maintaining efficient and compliant intake operations across their growing network.

Job Responsibilities

  • Monday – Friday (Standard Business Hours)
  • Oversee Daily Intake Operations Including Referrals, Admissions, and Patient Onboarding
  • Supervise and Lead Intake Staff to Ensure Timely Start of Care
  • Manage Intake Logs, Reports, and Patient Flow Tracking
  • Communicate with Referral Sources to Clarify Patient
  • Needs and Coordinate Care
  • Collaborate with Branch Directors and Business Development Teams
  • Ensure Accurate and Timely Data Entry into EMR Systems
    Monitor Compliance with PDGM Guidelines and Documentation Standards
  • Assist with Strategic Planning, Budgeting, and Process Improvement Initiatives
  • Participate in Quality Assurance and Performance Improvement Activities
  • Act as a Liaison Between Intake, Clinical Teams, and Executive Leadership

Job Requirements

  • Experience in Home Health Required
  • Leadership or Supervisory Experience Required
    Strong Knowledge of Intake Processes, Referrals, and Admissions
  • Experience with EMR Systems and Healthcare Documentation Standards
  • Familiarity with PDGM Guidelines Preferred
  • Strong Organizational and Communication Skills
