Job description

Intake Coordinator Responsibilities:
  • Manage the Home Health referral and intake process from initial referral through acceptance and transition to scheduling.
  • Receive and process new patient referrals from hospitals, skilled nursing facilities, rehabilitation facilities, physician offices, and other referral sources.
  • Review referrals for completeness and ensure all required patient, clinical, insurance, and physician information has been received.
  • Review Home Health orders and supporting documentation to identify requested services, including Skilled Nursing, Physical Therapy, Occupational Therapy, and other ordered disciplines.
  • Confirm patient demographics, contact information, insurance information, physician information, diagnosis, requested services, and other required referral details.
  • Obtain missing documentation, orders, clinical records, or additional information from referral sources as needed.
  • Communicate directly with referral sources regarding referral status, missing documentation, acceptance, or barriers to admission.
  • Coordinate with clinical leadership to determine whether the agency can safely and appropriately accept the patient based on clinical needs, service area, staffing availability, and required disciplines.
  • Collaborate with the Scheduling team to facilitate timely Start of Care (SOC) once a referral has been accepted.
  • Track pending referrals and follow up on outstanding information to prevent unnecessary delays in admission.
  • Maintain accurate and timely referral information within the company's EMR and referral management systems.
  • Ensure referral documentation is uploaded, organized, and available to the clinical team prior to Start of Care.
  • Communicate important clinical or operational information to the appropriate nursing, therapy, scheduling, and leadership teams.
  • Provide professional and timely communication to patients, families, physicians, referral partners, and internal team members.
  • Support an efficient handoff between Intake, Scheduling, and Clinical Operations to ensure a positive patient experience.
  • Maintain patient confidentiality and comply with HIPAA, company policies, and applicable Home Health regulations.
  • Assist with referral tracking, reporting, and other administrative responsibilities as needed.

Qualifications and Requirements:
  • 1+ year of healthcare intake, referral coordination, scheduling, insurance verification, or related healthcare administrative experience preferred.
  • Home Health or Home Care experience strongly preferred.
  • Previous experience processing Home Health referrals and admissions is highly preferred.
  • Understanding of Home Health services, including Skilled Nursing, PT, OT, and other clinical disciplines preferred.
  • Familiarity with Home Health orders, physician documentation, referral documentation, and Start of Care processes preferred.
  • Experience communicating with hospitals, physician offices, facilities, and other healthcare referral sources preferred.
  • Strong organizational skills and ability to manage a high volume of referrals and competing priorities.
  • Strong attention to detail with the ability to identify incomplete or missing referral information.
  • Excellent verbal and written communication skills.
  • Strong follow-up and problem-solving skills.
  • Comfortable working in multiple systems and learning Home Health EMR and referral platforms.
  • Ability to work collaboratively with clinical leadership, schedulers, nurses, therapists, and other operational teams.
  • Ability to work effectively in a fast-paced healthcare environment while maintaining accuracy and professionalism.
