Job description

Job Purpose:

EHM Senior Solutions is excited to welcome a new high energy, friendly, customer focused person as our Intake and Scheduling Coordinator to our Shared Services Home Health Division!

This individual is responsible for managing all aspects of the patient intake process, establishing, and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, managing the insurance verification and authorization and scheduling of patients with the intake team.

Key Responsibilities:
- Serve as the first point of contact for clients, providing professional and courteous service.
- Conduct intake assessments to gather necessary information from clients and determine their needs.
- Schedule appointments and coordinate with staff to ensure timely service delivery.
- Maintain accurate and up-to-date records of client interactions and appointments.
- Communicate effectively with clients, caregivers, and healthcare professionals to facilitate smooth service operations.
- Handle inquiries and resolve scheduling conflicts or issues promptly.
- Assist in the development and implementation of scheduling procedures and policies to improve efficiency.
- Collaborate with other departments to ensure comprehensive client care and service delivery.
- Provide administrative support as needed to ensure the smooth operation of the intake and scheduling processes.

Hours:
Half-time: Monday, Thursday, & Friday approximately: 8:00am - 4:30pm with rotating weekends and holidays.

This is not a remote/work from home eligible position. However, work from home will be required for rotating weekend on call.

Required Education:
- High school diploma or equivalent required.
- Associate's degree in healthcare administration or related field preferred.
Preferred Experience:
- Minimum of 2 years of experience in a healthcare or senior care setting, preferably in intake or scheduling roles.
- Proven experience in managing client or patient schedules efficiently and effectively.
- Experience with electronic health records (EHR) systems and scheduling software.
Required Skills and Abilities:
- Strong organizational skills with the ability to multitask and prioritize workload in a fast-paced environment.
- Excellent communication skills, both verbal and written, to interact with clients, families, and healthcare professionals.
- Proficient in using Microsoft Office Suite and other relevant software applications (EPIC, Allscripts, WellSky, Kinnser, My Ability).
- Ability to maintain confidentiality and handle sensitive information with discretion.

- Must have intake experience with home healthcare and/or hospice admission.
- Detail-oriented with a high level of accuracy in data entry and record-keeping.
