Overview

Care Hospice is currently seeking a Hospice Intake Coordinator to join our team.  If you are looking for a career with a purpose and a supportive team culture, then this is the position for you!

The Hospice Intake Coordinator is responsible for all aspects of the patient intake and admission processes including directing and coordinating and initiation of hospice services, and any other equipment, supplies, external services for hospice clients as prescribed by the physician, and/or requested by the client/family. Hospice Intake Coordinators respond to all incoming and outgoing physician and payer communication (insurance verification and authorization), and are in frequent communication with hospice clinical staff. Additionally, the Hospice Intake Coordinator assists the Admissions Nurse by following the patient throughout the referral-intake-admission process to facilitate a smooth transition to the hospice program.

Schedule: Monday - Friday, 8 am - 5pm

Territory: Arizona, California, New Mexico and Colorado

Responsibilities
  • Processes the referral and correctly collects all data possible from referral sources to ensure efficient and pre-qualifying of patients.
  • Ensures maximum third party reimbursement through direct oversight and coordination of care with multiple service providers.
  • Accurately documents all referrals/admissions activity into the EMR database and maintains spreadsheets to track referral activity.
  • Triages referrals appropriately to ensure timely access to hospice for all potential patients.
  • Ensures that all admission paperwork is received, reviewed and information updated in the database
Qualifications
  • High School graduate or equivalent required. (RN or LVN preferred but not required)
  • Minimum one (1) year of Intake experience in long term care environment, home health or hospice setting required
  • Excellent communication skills, including public speaking, both verbal and written.
  • Knowledge of corporate business management, governmental regulations, CHAP's standards, and private payor practices.
  • Experience working in Microsoft Office Suite and CRM & EMR systems

$25.00 - $26.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate's relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
