Paragon Intake Clinician - BioPlus Specialty Pharmacy

Location: This role requires associates to be in-office 1 - 2 days per week, fostering collaboration and connectivity, while providing flexibility to support productivity and work-life balance. This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace. Alternate locations may be considered if candidates reside within a commuting distance from an office.

TheParagon Intake Clinicianwill be responsible for ensuring that intake information is accurate and complete. Responsible for entering patient demographics into computer database according to company policy and assisting in setting up any and all home services required for new referrals.

How you will make an impact:

  • Obtains patient referral data as quickly as possible.
  • Documents initial referral information.
  • Notifies pharmacy and reimbursement managers of pending referral.
  • Enters new patient information into computer system completely and accurately.
  • Generates and tracks required forms - AOB, ABN.
  • Forwards all patient documentation to clinical staff in appropriate location and to reimbursement staff.
  • Communicates with patient/caregiver all applicable information regarding therapy and financial arrangements.
  • Assists in effective communication between the patient and the company, including documentation of suggestions, complaints, and requests.
  • May require extensive travel to worksite and other locations as necessary.

Minimum Requirements:

  • Requires a HS diploma or equivalent and a minimum of 2 years nursing experience in a clinical specialty involving infusion therapy, including patient assessment skills, venous access skills, and emergency patient care.
  • Requires a current, active, valid and unrestricted LPN/LVN, or RN license to practice as healthcare professional within scope of license in applicable state(s) or territory of the United States.

Preferred Skills, Capabilities and Experiences:

  • Minimum of 1 year previous specialized home health experience preferred.
  • Prior IV therapy/infusion experience preferred.
  • Certification as a Case Manager is preferred.
  • Bilingual preferred.

For URAC accredited areas, the following professional competencies apply: Associates in this role are expected to have strong oral, written and interpersonal communication skills, problem-solving skills, facilitation skills, and analytical skills.
