Long-term Care facility is Seeking an experienced Central Intake/ Admissions Coordinator!

Responsibilities:
· Your primary goal is to coordinate the central intake / admissions process and support census growth.
· You will work closely with our external marketing and admission liaisons.
· Provide clients (prospective residents, their families and referral sources) with facility and program specific information.
· Responsible for the clinical review of patient information. Must be able to review incoming referrals for acceptance
· Evaluate and assess potential patients for clinical appropriateness and placements into one of our family of centers
· Help to ensure proper admission documentation at the building level.
· Participate in the facility specific sales and marketing team events and plans as needed.
Qualifications/Requirements:
· PRIOR EXPERIENCE IN AN ADMISSIONS COORDINATOR, DIRECTOR OR MARKETING POSITION IN A SKILLED NURSING FACILITY OR REHAB CENTER IS A MUST.
· Associates degree or higher is preferred
· Solid computer skills, including Microsoft Office products and Point Click Care (PCC)
· Working knowledge of federal & state regulations and reimbursement (Managed Care, Medicare, and HMOs)
· Working knowledge of medications, diagnosis, with a clinical backround and the ability to review medical patient information
