Job Summary
We are seeking a Care Coordinator to join our team. In this role, you will work collaboratively with our clients to determine their home care needs, develop the best course of action, and oversee their care plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about home care practices. 
 
Responsibilities 
  • Collaborate with referral sources, clients (potential and existing), families, and healthcare staff
  • Coordinate a variety of home care programs
  • Develop individualized care plans
  • Educate patients on their home care options
  • Create goals and monitor progress toward goals
  • Recruit and train staff
  • Create schedules 
  • Participate in marketing events
Qualifications
  • Previous experience as a Care Coordinator or in a similar position is preferred
  • Certification as a medical assistant is desireable
  • Strong problem-solving and organizational skills
  • Ability to manage multiple projects or tasks and prioritize appropriately
  • Ability to work in fast-paced situations and make sound decisions quickly
  • Excellent interpersonal skills and high level of compassion
  • Strong verbal and written communication skills
