Position Title:ย Home Health Manager

Overview:ย The Home Health Manager is responsible for maintaining standards of resident care, and the case management of all Home Health Residents in the Independent Living units.

Essential Job Functions:ย 

  • Responsible for case management coordination for independent living residents.
  • Keeps physician and family aware of all changes in Home Health residentโ€™s overall health and well-being.
  • Maintains accurate documentation of medication administered and care rendered to Home Health residents.
  • Coordinates and books all lab work, doctorโ€™s orders, doctor appointments and delivery of resident medication.
  • Assists IL Community with Alert Response Procedure for both Emergency and non-emergency response occurrences.
  • Provides supervision and training to all Home Health associates.
  • Responsible for month-end reconciliation of Home Health charges and billing.
  • Completes and submits the schedules and payroll for the Home Health Department as required by organizational timeframes.
  • Responsible for on-call duties โ€“ carrying after hourโ€™s cell phone every other weekend.
  • Assists in direct resident care as necessary.
  • All other duties as assigned within the scope of the job functions.

You Have:

  • At least six months experience in a geriatric setting.
  • Current CPR certification.
  • Knowledge of billing of supplies and services.
  • Knowledge of invoice data entry.
  • Computer skills.
