About the Opportunity

InHome Home Healthย ย is seeking a highly qualified and experiencedย Registered Nurse (RN) โ€“ Home Health Managerย to join our organization in Indianapolis, Indiana. This leadership role is responsible for directing home health nursing services, ensuring the delivery of high-quality patient care, and supporting clinical excellence across the care team. The ideal candidate will bring strong clinical expertise, sound leadership capabilities, and a commitment to advancing positive patient outcomes in a home health setting.

Key Responsibilities

- Provide leadership and oversight of home health nursing operations to ensure the delivery of high-quality, patient-centered care.

- Conduct comprehensive patient assessments and develop individualized care plans aligned with clinical needs and established standards of care.

- Supervise, coach, and support nursing staff while ensuring compliance with applicable healthcare regulations, policies, and procedures.

- Collaborate with physicians, case managers, and interdisciplinary team members to coordinate care and optimize patient outcomes.

- Monitor patient status and adjust care plans as appropriate to support effective and timely treatment interventions.

- Ensure accurate, timely, and complete clinical documentation in accordance with regulatory and organizational requirements.

- Provide education to patients and families regarding health conditions, treatment plans, and home care management strategies.

Qualifications

- Current and activeย Registered Nurse (RN)ย license in the state of Indiana.
