Work Environment

The role is an onsite position, predominantly in the homes of our clients across Winchester and surrounding areas. You will work collaboratively with a skilled interdisciplinary team of healthcare professionals, including nurses, social workers, and case managers, to ensure every client receives integrated support tailored to their specific condition and environment. This dynamic home care environment allows for creativity and adaptability in delivering therapy, as you will evaluate and utilize the clientโ€™s home setting to enhance their rehabilitation experience.

Key Responsibilities
  • Develop and implement individualized physical therapy treatment plans that focus on preventing disability, relieving pain, and restoring muscle function.
  • Evaluate clients' physical abilities through diagnostic assessments, including functional movements and rehabilitation assessments, while adapting plans as necessary.
  • Supervise and mentor Physical Therapy Assistants to ensure adherence to the care plan and provide guidance on best practices in therapy delivery.
  • Conduct assessments of the client's home environment to ensure safe and effective rehabilitation, recommending modifications as necessary.
  • Administer therapeutic exercises, manual treatments, and use of physical agents according to prescribed plans.
  • Engage in active communication with clients and their families, instructing them on home exercises, equipment use, and overall physical health.
  • Compile detailed clinical records that include evaluations, care plans, and progress notes, ensuring adherence to compliance regulations.
  • Participate in interdisciplinary case conferences and provide valuable input on the overall treatment strategy for each client.
  • Maintain timely submission of all documentation and communication regarding clinical status and compliance with branch policies.
Patient Population / Client Focus

Your work will directly impact a diverse range of patients, from individuals recovering from surgery or injuries to elderly clients requiring ongoing rehabilitation. Our patients often have complex conditions that necessitate a compassionate and customized approach to care. Qualifications
  • A Doctorate degree in Physical Therapy (DPT) is required.
  • Current, valid PT licensure in the state of Virginia.
  • At least 2 years of experience working as a physical therapist in an acute care or home care setting.
  • Current CPR certification is mandatory.
  • Strong interpersonal communication skills and a passion for enhancing patient quality of life.
