This position is designed for those who thrive in an onsite home care setting, where you will work directly in clients' homes rather than in a traditional clinical environment. You will collaborate closely with interdisciplinary teams, including physical therapy assistants, case managers, and other healthcare professionals to ensure coordinated care and optimal patient outcomes.

• Develop and implement individualized physical therapy treatment plans designed to prevent disability, alleviate pain, and restore optimal physical function in accordance with prescribed care plans.
• Supervise the activities and interventions provided by physical therapy assistants, confirming adherence to treatment protocols.
• Conduct comprehensive evaluations of client physical capabilities through diagnostic assessments of muscles, nerves, joints, and overall functional abilities.
• Evaluate home environments to identify barriers to rehabilitation and make recommendations to enhance client autonomy and independence.
• Deliver hands-on physical therapy interventions, including therapeutic exercises, manual therapy techniques, and the application of physical agents.
• Instruct and motivate clients in exercise regimens, ensuring their engagement and adherence to rehabilitation protocols.
• Consult with physicians regarding client progress, adjustments to therapy, and synchronizing care plans based on observed outcomes.
• Maintain precise clinical documentation, including initial evaluations, progress notes, and discharge summaries to ensure compliance with regulatory standards.
• Engage actively in case conferences and collaborative care planning to integrate physical therapy goals with the comprehensive care objectives for each client.
• Educate clients and their families on the use of therapeutic devices and home exercise programs to facilitate self-management of their health conditions.
• Participate in ongoing professional development and training to remain current with best practices in physical therapy and home healthcare.

You will primarily work with diverse populations across various age groups who are managing health conditions that limit their physical functioning. This includes elderly clients seeking to maintain independence, individuals recovering from surgeries, and those with chronic conditions requiring ongoing therapy. Your role will be essential in motivating these individuals to achieve their therapy goals while addressing their unique challenges in a home setting.

• Doctorate Degree in Physical Therapy (DPT) is required.
• Current and valid Physical Therapy license in the State of New Jersey.
• One or more years of experience as a physical therapist, preferably in home health or acute care settings.
• Current CPR certification.
• A valid driver’s license and reliable automobile insurance to facilitate home visits.
• Demonstrated ability to communicate effectively with clients, families, and interdisciplinary teams.
• Commitment to clinical excellence and high standards of patient care.

Previous experience in home healthcare settings will be an advantage, especially familiarity with OASIS documentation and care planning processes.
