What You'll Do

As the Clinical Manager, you'll provide leadership and oversight for all clinical operations while ensuring outstanding patient care and regulatory compliance.

Responsibilities include:

  • Lead, coach, and mentor Home Health clinicians.
  • Ensure compliance with Medicare Conditions of Participation (CoPs), state, and federal regulations.
  • Oversee patient care, clinical documentation, and quality outcomes.
  • Drive Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Coordinate interdisciplinary care to ensure exceptional patient outcomes.
  • Support clinicians with assessments, care planning, and clinical decision-making.
  • Maintain high standards of regulatory compliance and documentation accuracy.
  • Promote infection prevention, patient safety, and clinical best practices.
  • Foster a culture of accountability, teamwork, and compassionate care.
Qualifications Required
  • Current Registered Nurse (RN) License
  • Minimum 2 years of Home Health Management experience
  • Strong knowledge of Medicare Home Health regulations and Conditions of Participation
  • Current CPR Certification
  • Valid Driver's License
  • Valid Auto Insurance
  • Excellent leadership, communication, and organizational skills
Preferred
  • HomeCare HomeBase (HCHB) experience
  • QAPI and Performance Improvement experience
  • Experience leading interdisciplinary clinical teams
