The Home Health Registered Nurse uses an individualized patient care plan to provide top quality care to all of our patients; keeping them Happy, Healthy and at Home. We are seeking a passionate RN Case Manager for our weekday team in Pender County and surrounding areas.
This position is Full-Time Monday- Friday 8am-5pm. Full benefits including: Medical, Dental, Vision, 401k with matching, and immediate PTO accrual.
RN Case Managers visit 5-6 patients on any given shift, driving to the patients home providing skilled nursing services. This position is primarily responsible for the initial assessment and case development of patients in all stages of life in their homes. Duties include but not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient's age and diagnosis.
Registered Nurses (RNs) with strong assessment, skilled nursing and documentation skills are strongly encouraged to apply!

We are 100% EMR and clinicians document in the patients' homes with company-issued laptops. RNs must be okay with traveling throughout the service area!
