Non-Clinical Intake Coordinator - Home Health Services (Boynton Beach Fl,)
Position Summary
The Non-Clinical Intake Coordinator plays a critical role in the patient onboarding and referral process. This non-clinical position is responsible for managing incoming referrals, verifying documentation, coordinating with referral sources, and ensuring patients are appropriately processed for admission into home health services.
The ideal candidate is highly organized, able to multitask in a fast-paced environment, and possesses excellent communication and customer service skills.
Responsibilities
  • Receive and process incoming home health referrals for Medicare skilled services
  • Communicate with hospitals, physicians' offices, case managers, rehab facilities, and referral sources
  • Review referral documentation for completeness and accuracy
  • Verify insurance information and coordinate intake documentation
  • Upload and manage patient records within the EMR system
  • Coordinate Start of Care scheduling with clinical staff and intake team
  • Maintain timely communication with patients, families, and healthcare partners
  • Ensure compliance with agency policies, Medicare guidelines, and documentation requirements
  • Assist with tracking referrals, pending documents, and admission status updates
  • Support office operations and collaborate closely with the clinical and scheduling teams
Qualifications
  • Previous home health intake, healthcare coordination, medical office, or customer service experience preferred
  • Knowledge of Medicare home health services is highly preferred
  • Strong organizational and multitasking skills
  • Excellent phone etiquette and communication skills
  • Experience with EMR systems preferred (Kinnser/WellSky experience is a plus)
  • Ability to work independently and in a fast-paced office environment
