Position Overview
We seek a dedicated Home Health Intake Coordinator to manage patient referrals and facilitate smooth transitions into our care programs. This role requires someone who is organized, compassionate, and able to coordinate effectively with healthcare providers, patients, and families to ensure all necessary documentation is completed accurately and efficiently.

Responsibilities

  • Process new patient referrals and gather required clinical information

  • Verify insurance benefits and eligibility for home health services

  • Coordinate initial assessments and facilitate admission paperwork

  • Communicate with healthcare providers, patients, and families

  • Maintain accurate and complete patient records

  • Schedule appropriate care staff based on patient needs

  • Ensure compliance with healthcare regulations and company policies

  • Track referral sources and assist with relationship management


Qualification

  • High school diploma required; Associate's degree in healthcare or related field preferred

  • Experience in healthcare administration, especially in home health settings
