Location: Portland, OR
Job Description:
Embark on an exciting remote, weekday pharmacist role that empowers you to deliver high-quality inpatient care from a flexible, home-based setting. As part of the clinical team, you will provide comprehensive pharmacotherapy services, lead medication therapy management, and contribute to population-based disease management and policy development. You will conduct drug utilization reviews, support prior authorizations and care coordination, and craft patient-specific medication action plans while communicating recommendations clearly to physicians, nurses, and other providers. This position offers ongoing professional growth, opportunities to educate clinicians and trainees, and a role in quality improvement and cross-disciplinary committee work that advances safe, effective medication use.
Requirements:
  • Doctor of Pharmacy (PharmD) degree
  • PGY-1 Pharmacy residency with 1 year of relevant practice experience
  • 3+ years managed care experience including prior authorizations, audits, appeals, ALJ hearings
  • Oregon Pharmacist License required
  • Oregon license required; Washington or California license acceptable
