CareRx is looking for a Full Time Clinical Consultant Pharmacist for a 12-month contract to join our Calgary team. The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities.ย The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services.ย

Role Accountabilities:

  • Act as aย liaisonย betweenย long term care homesย and retirement communitiesย and CareRx Pharmacyย to ensure optimal Resident care, Pharmacy Services, and Clinical support
  • Travel to LTC homes and retirement communities
  • Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/communityย leadership teams and pharmacy teams to develop/maintain strong partnerships
  • Complete medication reviews in currently serviced retirementย communities and long-term care homesย serviced by CareRx Pharmacy
  • Complete all required documentation and billing processes as required by MOHLTC, Alberta College of Pharmacy (ACP), and other federal and provincial standards and regulations
  • Provide education to Residents and staff on various health-related topics including theย MedsCheckย program and attend community-based wellness fairsย and other eventsย as required
  • Conductย education sessions withinย Long Term Care Homes and Retirement Communities withinย scope of practice, as applicable (e.g. policy and procedure, web portal,ย eMAR, influenza protocol, etc.)
  • Support CareRx Pharmacy policies and procedures within retirementย communities and long-term care homes
  • Completeย medication audits on-site in LTC Homes and Retirement Communitiesย as required
  • Ensure regular on-site visits are made to all assigned clientsย 
  • Attend/participate in annual professional conferences and trade shows, when applicable
  • Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable
  • Be an education, policy and pharmacy initiative resource
  • Be available for special clinical team projects as requiredย 
  • Complete/collaborate in the develop of medication audit follow-up improvement plans when applicable
  • Works collaboratively with other team members to support applicable homes and communities
  • Participate in pharmacy-related transition processes (e.gย new client transitions,ย eMARย transitions, etc.)
  • Help with pharmacy operations/drug distribution when required
  • Attend and actively participate in clinical team meetings

What You Will Bring to The Team:

  • Insured and currently licensed in good standing with the Alberta College of Pharmacy (ACP)
  • Proven ability to foster strong partnerships with other members of the healthcare team
  • Effective organization, planning, and time management skills
  • Knowledge of Kroll Windows applicationย and Microsoft Office programs
  • Completeย understanding ofย provincial regulations governing LTC homes and Retirement Communities
  • Must be familiar with the provincial standards of practice established for LTC pharmacies
  • Excellent customer service and interpersonal skills, oral and written communication skillsย and be able to interact with persons of varying levels of professional expertise
  • Excellent time management skills
  • Skilled in critical thought processes and be able to prioritize appropriately
  • Must demonstrate initiative and creativity

Location: Location will primarily be LTC homes and retirement communities in the Calgary and surrounding area

Work Shifts: This is a full-time position with work schedule focused within the standard Monday to Friday 9am-5pm.

Compensation Range: $103,000 - $115,000/year. Actual pay rate will be based on the individual candidateโ€™s credentials and experience.
