Job description

Job Summary:
Mount Sinai Hospital is seeking a dedicated Overnight Inpatient Pharmacist to provide essential pharmacy services during overnight hours. As the sole on-site pharmacist during this shift, you will play a critical role in ensuring the safe and effective use of medications for hospitalized patients. This position is vital to maintaining 24/7 pharmacy operations, supporting emergency care, and preventing delays in medication therapy.
The overnight pharmacist will be responsible for medication order verification, STAT order processing, emergency response support, and clinical decision-making in collaboration with physicians, nurses, and other healthcare professionals. The ideal candidate is a self-motivated, detail-oriented pharmacist with strong critical thinking skills and the ability to work independently in a fast-paced hospital environment.
Key Responsibilities:
• Verify and process medication orders in a timely and accurate manner, ensuring compliance with hospital protocols and safety standards.
• Manage STAT orders, code situations, and emergency medication requests, collaborating with the medical team to provide immediate pharmaceutical support.
• Monitor high-risk medications, including anticoagulants, controlled substances, and chemotherapy, to prevent adverse drug events.
• Provide drug information and clinical recommendations to physicians, nurses, and other healthcare providers.
• Oversee medication preparation and distribution, ensuring compliance with USP 797/800, sterile compounding guidelines, and hospital policies.
• Conduct medication reconciliation for newly admitted patients to prevent errors and improve medication safety.
• Support antimicrobial stewardship efforts by evaluating antibiotic therapy and making appropriate interventions.
• Manage inventory and ensure the availability of critical medications overnight, including controlled substances.
• Document clinical interventions and pharmacy activities to support patient care and compliance initiatives.
• Serve as the primary pharmacy resource for all inpatient units, responding to urgent medication inquiries and optimizing therapy as needed.
Required Qualifications:
• Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution.
• Active Illinois pharmacist license in good standing.
• Previous hospital or inpatient pharmacy experience required.
• Strong knowledge of EPIC, Omnicell, and DoseEdge or similar pharmacy automation systems.
• Ability to work independently as the sole overnight pharmacist while maintaining high standards of patient care.
Preferred Qualifications:
• Completion of a PGY-1 pharmacy residency or equivalent clinical experience.
• Board Certification (e.g., BCPS, BCCCP) preferred but not required.
• Experience with sterile compounding, USP 797/800, and investigational drug services.
