*This is a fulltime Career Service (CS) Health Educator position working with the Department of Health in Lee County.*
This is a highly professional position with lead responsibilities for linking and re-engaging Human Immunodeficiency Virus (HIV) positive clients to care in the 7 counties of Area 8. This position will work with the current Human Immunodeficiency Virus/Acquired Immune Deficiency Syndromes (HIV/AIDS) and Sexually Transmitted Disease (STD) intervention staff to identify clients who are not in care. This position will work with the data to care team (D2C) from State health office, as well as with community partners in the identification of those out-of-care clients and those clients who have been released from the county jail. This position is classified as a sensitive position. The incumbent handles protected health information on a "need to know" basis following Department of Health, HIPAA, and Information Security policies, protocols, and procedures. Employees abide by State/Federal laws governing health information. Responsible to maintain confidentiality, security, privacy and integrity of protected health information.
Programmatic Duties:
Use available databases (CARE Ware, STARS, HMS, Provide, the Linkage module etc.) to re-engage and link HIV positive individuals who are not in medical care, not taking anti-retroviral medication (ARVs), or have tested positive but have not been engaged into care.
Contact individuals who are not in medical care, or who have been tested positive and are not aware of treatment options and community resources. The goal is to get the clients into medical care, initiate ARVs, and work collaboratively with case management support services to ensure continuum of care.
Attend Statewide Linkage conferences and local case management meetings. Participate in Quality Assurance and Quality Improvement initiatives.
Work with the local county STD Department to ensure that HIV and STD positive inmates are linked to care upon release from the county jail. Work closely with Area 8 surveillance officers to gain information on newly diagnosed HIV+ individuals, Area 8's medical and non-medical case management staff to identify and notify clients who are out of care from their case management database. This will also involve working with the Area Planning Groups such as the Regional HIV/AIDS Consortia (RHAC), the Prevention Planning Committee, the Continuum of Care Committee, and the Early Intervention Consultant (EIC) on patient care matters.
