Job description

Position Title: Registered Nurse (RN) - Case Manager

Location: Mission Viejo, California
Facility Type: Acute Care Hospital
Duration: 13 Weeks
Unit: Case Management


Schedule
  • Day Shift: 08:00 AM - 04:30 PM
  • 5x8-hour shifts
  • 40 hours per week
  • Weekend rotation required (every other weekend)

Pay Rate
  • Local: Up to $42.00 - $52.00 per hourย 
  • Travel: Up to $60.00 - $70.00 per hour

Position Overview

"Navitas Healthcare, LLC" isย an experienced Registered Nurse Case Manager to support care coordination, discharge planning, and utilization management in an acute care hospital setting. The RN Case Manager will work closely with interdisciplinary teams to ensure safe, efficient, and timely patient transitions while optimizing clinical and financial outcomes.


Key Responsibilities
  • Conduct patient assessments and develop individualized care plans
  • Coordinate discharge planning including SNF placement, DME, and transportation
  • Collaborate with interdisciplinary teams for care coordination
  • Participate in family meetings and care conferences
  • Ensure compliance with CMS, HIPAA, Joint Commission, and hospital policies
  • Support appeals, denials, and utilization review processes
  • Coordinate complex discharges involving social, behavioral, and medical needs
  • Educate patients and families on care plans and post-discharge resources
  • Maintain accurate and timely documentation in Epic EMR
  • Work with providers to optimize patient outcomes and reduce length of stay

Required Qualifications
  • Minimum 2 years of acute care Case Management experience
  • Strong experience in discharge planning and care coordination
  • Ability to independently arrange SNF placement, DME, and transportation
  • Experience in hospital-based case management setting

Education
  • Graduate of an accredited Nursing program (ADN or BSN)

Licensure / Certification
  • Active California RN License (required)
  • BLS certification preferred

Required Skills & Experience
  • Epic EMR proficiency (required)
  • Strong knowledge of insurance processes (HMOs, IPAs, authorizations)
  • Experience with adult and geriatric patient populations
  • Strong understanding of community resources and post-acute care options
  • Ability to manage high caseloads (approx. 1:25 ratio)
  • Weekend rotation flexibility
  • Strong communication, problem-solving, and organizational skills
  • Knowledge of CMS, DRG, HIPAA, and regulatory compliance standards
