The W Assisted Living Communities is seeking a compassionate and dedicated Case Manager / Financial Case Manager. We have our eye out for a candidate with a proven track record of successfully coordinating services for residents and managing financial/benefits processes, while remaining committed to excellence in resident satisfaction. The chosen candidate will have the opportunity to work with a positive and supportive team of healthcare professionals.

Job Responsibilities:

  • Maintain firm adherence to the resident's "Bill of Rights," as they relate to the facility

  • Communicate with the resident's care providers

  • Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals

  • Coordinate and supervise clinical care for residents

  • Maintain clear and concise documentation in accordance with facility requirements

  • Manage financial case management duties, including Medicaid applications, recertifications, and benefits coordination

  • Track and follow up on pending financial cases to ensure timely resolution

  • Coordinate with families, financial institutions, and government agencies regarding resident benefits and entitlements

  • Monitor resident accounts and liaise with billing/business office on financial status changes

Qualifications/Requirements:

  • Prior experience in Long Term Care is a PLUS or 1 year of experience as a Case Manager required

  • Experience with Medicaid applications and financial case management strongly preferred

  • Previous ALIS experience is a plus

  • Excellent phone skills and ability to work with families/other care providers

  • Strong organizational skills, passion and compassion for the population

  • Ability to work cooperatively with multidisciplinary team

  • Comfortable managing both clinical case management and financial/benefits coordination responsibilities
