Job description

Position Summary
The Medical Assistant - Medical Records Clerk has dual role in role in supporting back office clinical and administrative operations to ensure smooth and efficient processing of patient healthcare. Administrative duties include handling and maintaining patient medical records with confidentiality, accuracy, and respect at all times, as well as ensuring that all medical information is filed correctly and up-to-date for the member's continuity of care. Clinical duties include back office processing and patient care-related services under the direction of the professional staff.

Duties and Responsibilities
  • Input new patient information, visits/consult notes, lab results/tests, correspondences into Electronic Health Record (EHR) system and appropriate medical records.
  • Maintain patients' medical records to ensure organized and updated both electronically and hard copy
  • Route orders, reports, and requests to the appropriate department and providers
  • Audit medical record for missing documentation, signatures, errors, and omissions
  • Identity duplicate records or improperly filed records and resolve immediately by following established procedures
  • Process all requests for medical records from patients and providers by following established procedures
  • Verify authorization for release of records/information to ensure accuracy and validity of the request before release of records/information
  • Track and log all disclosures to maintain compliance with HIPPA and privacy laws
  • Interviews and exits patients, completing appropriate medical agency and consent forms, ensuring patient understanding and obtaining signatures.
  • Document patient intake in the Electronic Health Record (EHR), which includes chief complaint, medical history, social history, vitals, and appropriate tests/labs performed and immunizations administered.
  • Perform phlebotomy, injections, vitals if deemed by the Medical Practitioner, including but not limited to visual acuity, audiometric screening, venipuncture, injections, and specimen handling
  • Perform Point-of-Care testing, such as Blood Glucose, Urinalysis, EKG, HbA1c, INR, Blood Chemistry, Pregnancy Test, Bilirubin, and other standard primary care back office procedures.
  • Assist Medical Practitioners (i.e. physicians, PA-C, NP) with patient examinations and procedures as directed
  • Coordinate interdisciplinary and integration of primary care medical services with in-house Dentistry, Optometry, Endocrinology, Mental Health services
  • Review patient medical record to ensure completeness and accuracy
  • Route appropriate medical orders, results, documentation and instructions to the appropriate personnel
  • Maintain and stock clinical inventory supply and dispensary
  • Maintain equipment for clean and operational use, as well as maintain calibration, sterilization, and repair/maintenance logs
  • Clean, stock and maintain exam rooms.
  • Follow OSHA and universal precaution protocols
  • Strict adherence to the policy and procedure manual
  • Provide or direct patients/individuals seeking information regarding programs and services offered by NECC
  • Other duties as assigned by Clinic Manager

Required Education/Experience and/or Licensure/Certification
โ€ข Medical Assistant Certificate or Certified Medical Assistant
โ€ข High School Diploma and/or GED equivalent preferred, but not required.
โ€ข Knowledgeable of the alpha-numeric system
โ€ข Commitment to goals and philosophy of Northeast Community Clinic
โ€ข Current CPR and/or BLS Certification
โ€ข Valid State Identification
Physical Requirements and Working Conditions
โ€ข OSHA Category 2 - Involves no regular exposure to blood, body fluids, or tissues, but may require unplanned tasks that involve exposure to blood, body fluids, or tissues.
โ€ข The work is majority of the time non-sedentary in nature.
โ€ข The employee is regularly required to communicate, frequently required to use repetitive motions, move, remain stationary, regularly push, pull and lift up to 20 pounds and occasionally push, pull and lift up to 40 pounds.
โ€ข Must possess mobility to work in a standard office/clinic setting (in some cases both settings) and to use standard office/clinic equipment, including a computer, stamina to maintain attention to detail despite interruptions, read printed materials and use a computer, and communicate in person and over the telephone.
