Job Summary:

Kadima Rehabilitation & Nursing at North Strabane is seeking a dedicated and compassionate Evening LPN to join our team at North Strabane Rehabilitation and Wellness Center in Canonsburg, PA. As an Evening LPN, you will be responsible for providing exceptional care to our residents while working closely with our interdisciplinary team to ensure seamless transitions and optimal outcomes.

Responsibilities:

  • Provide direct patient care, including vital signs, wound care, and administration of medications as prescribed

  • Collaborate with the interdisciplinary team to develop and implement individualized care plans

  • Conduct routine rounds to monitor resident condition and report any changes or concerns to the nurse manager

  • Maintain accurate and detailed records of patient care and treatment

  • Participate in quality improvement initiatives and staff development programs

  • Demonstrate professionalism, compassion, and respect for residents, families, and colleagues

Requirements:

  • Current LPN/LVN license in the state of Pennsylvania
