We are currently seeking an experienced:

HEALTH INFORMATION MANAGEMENT - MANAGER

JOB SUMMARY:

  • Due to the nature of the job, a high degree of confidentiality and ethics is required
  • Serves as a Department head
  • Participates in the daily room rounding program as assigned by the Administrator
  • Serves as Manager on Duty (MOD) based on Department Head rotation schedule, as assigned by Administrator
  • Attends all necessary meetings deemed appropriate by Administrator, including stand-up, weekly Medicare and monthly/quarterly QA.
  • Assigns ICD-10-CM coding upon new admissions, re-admissions and interim resident stay; entering accurate codes into the facilityโ€™s EHR system timely following acknowledgment.
  • Processes system Diagnosis Request, assigning ICD-10-CM codes based on Providerโ€™s signed documentation.
  • Maintains Medicare certifications, sending a monthly report to the LHRS HIM Directorย 
  • Communicates with outside agencies for collection of necessary documentation, including documentation that requires signature.
  • Maintains Provider Visit Log, to ensure each resident is seen timely by their Provider.
  • Completes required HIM clinical chart audits, reports findings to appropriate staff timely following audit completion and presents findings during monthly/quarterly QA meetings.
  • Submits a monthly trend summary report to the LHRS HIM Director
  • Audits the EHR system to ensure providers are signing e-orders and order reviews within the approved time frame
  • Maintains the EHR update of discontinued medications and UDAs upon resident discharges
  • Updates the monthly order review date within the EHR for new and readmissionsย 
  • Sets up baselines within EHR for nursing assessments
  • Maintains a DNR notebook at each nursing station.
  • Assist with scanning documentation into EHR system
  • Maintains any remaining, on-site and/or off-site storage of medical record charts.
  • Coordinates ARD process between facility and Corporate Management Audit & Appeals staff, including the copying and scanning of information.
  • Processes non-financial Release of Information request
  • Assist as needed with educating Providers on the EHR Practitioner Engagement to ensure they have access and understanding of system functions and signing requirements.
  • Completion of daily checks to ensure the laptop backup system is functioning and upload log quarterly
  • Maintains HIM Disaster Kit, including facesheet notebook
  • In case of fire or fire drill:
    • Shut off all electrical equipment within HIM office
    • Report to designated areas
  • In case of evacuation:
    • Reports to evacuation assembly areas and documents patient arrival
    • Pulls factsheet/diagnosis sheet binder from HIM office to identify residents
    • Prepare emergency packet, refer to LHRS Disaster Plan Policy
  • Performs other duties as assigned

JOB REQUIREMENTS:

  • RHIA/RHIT, preferred or 3 yrs Long Term Care Medical Records experience required
  • Must have High school diploma or equivalent
  • Must have computer knowledge and experience
  • Must have medical terminology knowledge
  • Must have Anatomy & Physiology knowledge
  • Must be able to work proficiently alone
  • Must be able to work well under pressure and problem solve
  • Must be able to collaborate well with other employees
  • Demonstrates neat appearance and good personal hygiene
  • Dresses with business attire appropriate for the work environment
  • Must read, know, and follow personnel, department, and facility policies
  • Must have a valid driverโ€™s license in the state of employment
