Responsibilities
  • PATIENT ASSESSMENT AND PLANNING:
    • Assess patient eligibility for specific service needs, developing individualized integrated action plans.
  • RESOURCE REFERRAL AND MONITORING:
    • Refer and connect patients to resources, monitoring adherence and providing necessary follow-ups.
    • Assist patients in accessing medical, social, education, and employment resources.
  • DOCUMENTATION AND REPORTING:
    • Complete detailed reports on needs assessment and case management activities.
    • Ensure timely and comprehensive input of all necessary case management data into the electronic patient health record.
  • PATIENT SUPPORT AND COORDINATION:
    • Provide crisis and de-escalation techniques.
    • Ensure patients have reliable transportation to BrightView and other care providers.
  • PROFESSIONAL DEVELOPMENT AND COMPLIANCE:
    • Attend required meetings, including supervision, treatment team meetings, and quarterly in-service.
    • Adhere to regulatory and company requirements in documentation and evaluation.
    • Participate in TCM audit, meeting assessment and treatment plan completion expectations.

KNOWLEDGE SKILLS, AND ABILITIES
  • Excellent verbal and written communication skills
  • Highly empathetic and compassionate to effectively support the recovery journey of BrightView's patients
  • Competent at working with a diverse population of colleagues and patients
  • Natural problem solver, looks for solutions to best meet patient needs with a sense of urgency
  • Team-player, able to work collaboratively in a multidisciplinary healthcare environment
  • Adaptable and agile within a dynamic work environment
  • Technologically capable, comfortable operating in multiple systems for communication and documentation purposes. Familiarity with MS Office software (Outlook, Teams, Word, etc) preferred
  • Embraces BrightView's culture of compliance - operates with a high degree of integrity and compliance to work standards and regulatory requirements
  • Familiarity with HIPAA and 42-CFR Part 2 desired
  • Prior experience with harm reduction a plus

Qualifications
EXPERIENCE
Required: 1 year of experience working with adults in a human service setting.
Preferred: Prior experience with harm reduction.
EDUCATION:
Required: Bachelors Degree in a related Behavioral Science field.
