Job Summary:

To work within Gándara Residential Services team model, establishing open communication, support, and accountability among workers and consumers. The case manager is accountable to the Program Director/Clinical Supervisor, who is responsible for providing supervision and training. The Case Manager staff is one who actively promotes the philosophy of Gándara Center, and maintains a high level of commitment to the program

Duties and Responsibilities:

  • To share all program expectations and responsibilities with DCII and DCI
  • To provide advocacy/case management services
  • To develop strong, supportive relationships with the clients, and provide for client care and safety.
  • To complete all clinical documentation and casework for assigned residents: including but not limited to: adult comprehensive assessment, individual action plans, group progress notes, weekly progress notes, discharge summary, and transition/discharge/aftercare planning.
  • To maintain updated clinical notes and reporting in accordance to established record keeping procedures.
  • To facilitate daily evidence based psychoeducation groups and other recovery based groups.
  • To provide weekly individual case management sessions with assigned residents
  • To administer and complete the EIM/ESM forms and adhere to funding sources requirements
  • To maintain direct supportive contact with residents to continually assess their individual needs, provide information, discuss problems, set limits and encourage growth and development.
  • To be capable of assuming enhanced responsibilities for planning and overseeing on and off unit activities.
  • To develop and maintain a working relationship and knowledge of other social service agencies in the community.
  • To notify supervisory staff immediately in the event of a client crisis, suspected illegal activity or any behavior constituting an incident in the program.
  • To participate in all case consultation meetings, staff meetings and case conferences.
  • To administer drug/alcohol screens to residents when considered clinically appropriate.
  • To provide orientation of services to prospective clients, family members and community interest groups.
  • To utilize de-escalation techniques to reduce, prevent, or resolve escalated resident conflicts
  • To arrange patient transportation to outside appointments when public transportation is not available or recommended.
  • To participate in training programs (both external and internal), regularly research materials relative to chemical abuse, mental illness and treatment approaches.
  • To acquire and maintain current the necessary certifications mandated by the Agency and funding sources.
  • To adhere to program and agency policies and procedures.
  • May be required to monitor resident medication and complete hourly checks as needed
