Core Responsibilities
The Case Manager coordinates services and referrals that support clients in meeting individualized treatment goals. This entry-level, full-time onsite role in Mt Vernon focuses on connecting individuals to resources and ensuring continuity of care.
Key responsibilities include completing intake-based needs assessments and routine follow-ups; documenting services and required data for client records and funding sources; collaborating with primary counselors to develop, implement, and monitor treatment plans and client progress; and coordinating ancillary supports such as housing, medical care, food, employment, and transportation through community partners.
Required qualifications/skills:
- High school diploma or equivalent
- Valid Ohio Driver License, two years of driving experience, and proof of full auto insurance
- Ability to pass a BCI check and post-offer/pre-employment drug screen
- CDCA credential preferred (or obtained within one year of hire)

A Typical Workday
Your day blends office-based coordination with community-based work. You'll start by reviewing referrals, prioritizing follow-ups, and organizing next steps for each client. Throughout the day, you'll communicate with clients and partner agencies, confirm appointments and resource availability, and update records to keep information current and accurate. Some days include in-county travel and transporting clients to support access to services. You'll also check in with the care team to align on progress and ensure plans remain on track, then close out the day by reconciling documentation and preparing for upcoming contacts.
