Job description

DESCRIPTION:

The Assessor, under the direction of the Clinical Director, plays a crucial role in evaluating clients' behavioral, mental health, and overall well-being. This position involves conducting comprehensive assessments, developing individualized care plans, and collaborating with a multidisciplinary team to ensure the highest quality of care for our clients.


JOB DUTIES:
1. Client Assessment:
- Conduct thorough and standardized assessments of clients' behavioral and mental health status.
- Utilize various assessment tools and techniques to gather comprehensive data on clients' history, symptoms, and functioning.


2. Individualized Care Planning:
- Develop detailed, individualized care plans based on assessment findings.
- Collaborate with clients, families, and other healthcare professionals to establish achievable goals and interventions.


3. Documentation and Reporting:
- Maintain accurate and up-to-date documentation of all assessments, care plans, and client progress.
- Prepare detailed reports and summaries for use in clinical team meetings and for compliance purposes.


4. Collaboration and Communication:
- Work closely with a multidisciplinary team including therapists, case managers, and medical professionals to coordinate care.
- Communicate assessment findings and care plans effectively to clients, families, and team members.


5. Continued Professional Development:
- Stay current with the latest research, trends, and best practices in behavioral and mental health assessment.
- Participate in ongoing training and professional development opportunities


QUALIFICATIONS
1. Education:
a. Master’s degree in Psychology, Social Work, Counseling, or a related field preferred.


2. Experience:
a. Minimum of 2 years of experience in behavioral health assessment or a related field.


3. Licensure:
a. Must hold a valid and current professional license (LCSW, LMFT, LPC, or equivalent) in the state of employment.


4. Skills:
a. Strong clinical assessment and diagnostic skills.
b. Excellent written and verbal communication abilities.
c. Proficiency in electronic health records (EHR) systems.
d. Ability to work independently and as part of a multidisciplinary team.
