Salary: $24.50-$27.50/hour
Schedule: 35 hours/week | Full-time | Hourly
Job Summary
This position provides person-centered case management services to individuals returning to the community from incarceration, as part of Delphi Rise's Monroe County Reentry Program. Our approach is grounded in dignity, respect, and belief in each person's potential. We walk alongside clients as they rebuild their lives, offering support with housing, employment, health care, and healing.
At Delphi Rise, we don't define people by their past. We focus on building trusted relationships that foster growth, reduce barriers, and strengthen community connections because we know that no one can do it alone. Through our reentry program, we support individuals in creating a future defined by stability, purpose, and hope.
Essential Duties and Responsibilities
Additional duties may be assigned.
โ€ข Manage a caseload of 40 to 50 clients, supporting access to benefits, employment, treatment, housing, childcare, healthcare, identification documents, and other critical services.
โ€ข Coordinate services in collaboration with the New York State Department of Corrections and Community Supervision (Parole), Monroe County Probation, local courts, and community partners.
โ€ข Maintain accurate and timely case notes and client records.
โ€ข Develop individualized service plans that align with each client's goals and needs.
โ€ข Facilitate or arrange support groups such as Cognitive Behavioral Therapy, parenting education, domestic violence support, and anger management.
โ€ข Conduct biweekly follow-up contacts with all assigned clients.
โ€ข Actively participate in Monroe County Reentry Task Force meetings.
โ€ข Provide services at the Probation Office and in Monroe County Jail 1-2x per week.
โ€ข Perform other duties as assigned by management.

This role does not include supervisory responsibilities.

While we strongly encourage applicants with lived experience in the criminal justice system, candidates cannot have been on probation within the past three years. Access to the Monroe County Jail is an essential part of this role, and all candidates must meet the jail's clearance requirements.

Physical Requirements

Environmental Conditions
Some off-site work may be required to meet with clients or attend community events. This may include exposure to inclement weather or travel to partner agency locations.
