About Us: Acacia Care LLC, a HCBS provider based in Phoenix, AZ, is expanding its team. We provide high-quality services to individuals with developmental disabilities.

As a Direct Service Professional, you will make a difference in someone's life every day. DSPs provide support to the individuals we serve to live as independently as possible. DSP's provide Rerpite, Habilitation, and/or Attendant Care Services.

Respite Provider

  • Duties include providing the family/immediate caregivers a break
  • Supervise member
  • Assist with member care
  • Provide activities (walks, games, community outings, etc.)

Attendant Care Provider

  • Duties include providing total care to members for basic daily living needs
  • Such as: transferring from bed to wheelchair
  • Personal care/bathing
  • Clothing
  • Feeding
  • and more

Habilitation Provider

  • This service is intended to enable individuals to acquire knowledge and skills
  • Support member to increase or maintain self-sufficiency
  • Provide training/assistance in essential activities required to meet personal and physical needs
  • Teach skills to promote health and safety

Qualifications:

  • Must be at least 18 years of age
  • High School Diploma or GED equivalent
  • 3 months of experience in human services
  • Email Capability
  • Reliable transportation
