Job description

Summary of Duties and Responsibilities:

Under the general supervision of the Director of Home and Community Based Services, and direct supervision of the HCBS Supervisor, the Assessor is responsible for assessing and coordinating an initial person-centered care plan (PCCP) and person-centered service plan (PCSP) within designated cost caps and referral to available community resources. Assessors assure that the initial services for any assessed program are appropriate and meet the needs of the client to live safely in a Home and Community Based (HCBS) setting.

For the NextGen MyCare Ohio Waiver program Assessors will conduct Initial Level of Care (LOC) assessments as well as LOC re-determinations and Significant Change in Condition Assessments for all consumers as requested through the Managed Care Organization Plans (MCOPs). The Assessor will utilize the assessment tool specific to MyCare, to provide a limited, detailed assessment to make an LOC determination.

Must have the following abilities to perform the responsibilities:

  1. Ability to effectively communicate and build rapport with the public, in person, by phone, and via email,
  1. Ability to comprehend written material related to essential job functions,
  2. Ability to accurately document facts related to essential job functions,
  3. Ability to comprehend, recall, and apply facts related to essential job functions,
  4. Ability to analyze, evaluate, and implement a reasonable course of action based on available information,
  5. Ability to lift/carry 20 lbs. upstairs,
  6. Ability to climb steps and access private homes/locations which may not be readily accessible.
  7. Ability to travel to community and consumer locations within our four-county service area (Ashtabula, Mahoning, Trumbull, and Columbiana counties), and
  8. The ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio.

Job Requirements:

  • The Applicant must have a Licensed Practical Nurse, or Social Work Trainee but will require oversight from the appropriate assigned licensed staff member.
  • Applicant shall possess the knowledge and skills necessary to provide the level of assessment required by the client and by any legal regulations.
  • OSHIIP certification must be obtained within one year of hire date.
  • Applicant must possess a valid driver's license and proof of automobile insurance.
  • Must pass a criminal background check and drug screen.
