HCBS Community Living Support (CLS) Care Provider Essential Job Functions

1. Provide habilitation and skill building to enable the beneficiary to acquire and maintain skills, which support more independence.

2. Provide training in interpersonal skills and development and maintenance of personal relationships.

3. Provide skill building to support the beneficiary in increasing community living skills such as shopping, recreation, personal banking, grocery shopping and other community activities.

4. Provide transportation to support implementation of in-home skill building.

5. Document service provision for Community Living Support and complete incident reports if warranted.


HCBS Community Network (CN) Care Provider Essential Job Functions

1. Provide individualized day activities that support the waiver beneficiary’s definition of a meaningful day in an integrated community setting, with persons who are not disabled.

2. Encourage the beneficiary to increase or maintain their capacity for independence and develop roles valued by non-disabled members of the community.

3. Promote participation in community life while developing natural supports within integrated settings.

4. Link beneficiary to adult education programs, community-based classes for development of hobbies or leisure/ cultural interests or volunteer work.

5. Accompany beneficiary when utilizing public transportation.

6. Document service provision for Community Network Care and complete incident reports if warranted.


Respite Care Provider Essential Job Functions

1. Provide periodic or scheduled support and relief to the primary care giver(s) from the responsibility of stress of caring for the individual.

2. Provide respite inclusive of overnight, weekend care or emergency care.

3. Administer pre-poured medication, preparing meals, assisting the person served with his/her routine, and cleaning up after activities and meals.

4. Engage the person served in leisure / social activities unless otherwise directed.

5. Provide your own transportation to and from the location of the person(s) who will receive services.

6. Document service provision for Respite Care and complete incident reports if warranted.


Additional Essential Job Functions

1. Always maintain a valid driver’s license and an “acceptable” driving record and be responsible for driving personal vehicles. Report all traffic citations to the personnel office immediately and in writing.

2. Must provide transportation to program participants utilizing their personal vehicle.

3. Provide own transportation to and from location of services.

4. Run goals based on the person served Individual Support Plan (ISP) under NC Innovations Services.

5. Provide quality supervision and interaction with the person served.

6. Keep the residential area and other training areas neat, clean, and safe.

7. Participate, provide, and assist with any programmatic and habilitative services and instructional programs as designated.

8. Attend staff meetings.

9. Keep abreast of all necessary safety and precautionary measures to ensure the well-being and safety of the person served.

10.Fill out accident/incident reports immediately following an incident, according to expectations.

11.Assess person served needs at least annually or as indicated.

12.Participate and maintain all NC Innovations training, in-service training, and other required training, as necessary.

13.Complete all NC Innovations documentation according to expectations and submit it by deadline.

14.Participate as a member of the interdisciplinary team as required.

15.Routinely lift to 40 pounds independently and routinely lift from 41 to 140 pounds with the assistance of one additional person.

16.Maintain confidentiality in all matters related to the program participant.

17.Perform other related duties as required.


Minimum Education/Experience Must be eighteen years or older and be a high school graduate (with a diploma) or have a GED Certificate. Preference will be given to those applicants who have experience working with individuals with developmental disabilities and mental health challenges.


Hours/Days of Work (non-exempt)

Periodic – Hourly as needed; Determined by person served(s) authorized hours.
