What You'll Do

As a Care Coordinator, you'll be the connection between families, providers, and services - ensuring members receive the highest quality care.

  • Manage and support a caseload of DDD members
  • Conduct home visits and build strong relationships with families
  • Coordinate Attendant Care, Habilitation, and Respite services
  • Advocate for member needs and ensure services align with PCSPs
  • Collaborate with DSPs, families, and support teams
  • Monitor service utilization, compliance, and outcomes

  • Group Home Management or Direct Support Professional (DSP) roles

  • Valid driver's license & reliable transportation
  • Ability to pass APS & CPS registry checks
