We are seeking a Mental Health Professional I Housing Coordinator/ACT Team for Williamsburg, Horry and Georgetown Countis who is motivated to help individuals and families navigate life's challenges and achieve long-term wellness.

This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Waccamaw Center for Mental Health, all clinical locations in Kingstree, Georgetown, Myrtle Beach and Conway. This position MAY be eligible for SCOMH Student Loan Repayment to cover some of the cost of your outstanding loan(s) balance associated with the completion of a healthcare degree that is relevant to the applicable position.

As a Mental Health Professional I/Housing Coordinator for the ACT Team you will be under limited supervision and serve as a WCMH's Housing Coordinator to ensure patient's access to appropriate community-based housing. This includes accessing patients' psychiatric status to determine the ability to move to a lower level of care in the housing continuum. Must have experience working with a chronically mentally ill population.

Responsibilities for the Mental Health Professional I/Housing Coordinator ACT Team: Access patient psychiatric status to determine the ability to move to a lower level of care in the housing continuum. Provide oversight for WCMH's housing programs, including Community Housing, Homeshare, Homeshare Respite, TLC/Housing and Homeless Program Supportive Housing Developments including Community Residential Care Facilities (CRCF's) and other supportive housing programs, as applicable. Coordinate with the agency's Transition Program Staff, including Clinical Care Coordinators (CCC's) and Transition Specialists, MCO CCC's, Certified Peer Support Specialists and Benefit/Entitlement Specialists.

Enter and maintain Center Homeshare/TLC program patient information in the agency's SharePoint Site and CIS/Electronic Health Record. Participate in other OMH or partner agency programs and activities as may be required. Attend treatment team meetings daily and provide consultation regarding complete cases as appropriate.

Complete documentation according to QA/SCOMH standards, Corporate Compliance policy and Risk Management plan. Ensure timely completion of prior authorizations for services. Attend all annual trainings to include safety, MAPS, Cultural Affairs and clinical related courses of instruction.

Demonstrate adherence to CARF guidelines and regulations. Minimum and Additional Requirements Master's degree in guidance, counseling or social services and two (2) years of post-graduate experience in a clinical counseling program. Additional Requirements: Must be able to meet OMH credentialing requirements which include specific graduate course work including Psychopathology, Psychopathology Diagnostics, Clinical Assessment - Practice with Family Therapy (DSM) and Medical Ethics.

Must be able to show evidence of clinical experience providing direct patient care. This experience maybe documented as a practicum, internship or post-graduate clinical experience. Ability to lift up to fifty (50) pounds; ability to see, hear at normal volume, stand, walk, bend, stoop and climb at least two (2) flights of stairs.

Flexible schedule is required and services are delivered in the community and in patient's home. Preferred Qualifications Bilingual abilities in English and Spanish (or another language) are a plus. Additional Comments Post hire, employee must: Be able to provide 10-year DMV driving record.

Be able to use a state vehicle for work related purposes. Be able to integrate and coordinate services with those of other disciplines and to communicate effectively. Be organized and communicative.

Be able to interpret and apply rules and regulations. Ability to maintain effective relations with clients, their families, other professionals and the public. Exercise good clinical judgement and be able to function in dependently while exhibiting self-direction.
