Job description

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. 

We are actively seeking Community Integration Counselors in the Marana & Saddlebrook, AZ areas. This role provides individualized services designed to assist participants in managing and overcoming the difficulties confronted by an individual with a disability living in the community.

The hourly rate is $15.00 per hour!

Responsibilities

  • Support and encourage individuals to increase their ability to be as independent as possible
  • Meet with waiver participants on a regular basis either in the individual’s residence, the community, or an office setting
  • Collaborate with the Case Manager to develop and facilitate programs which meet the social, emotional, physical and personal needs of the individual
  • Provide ongoing support for community integration efforts
  • Document individual’s progress toward acquiring community integration
  • Assist in building a support network between the individual and the community
  • Ensure medical consultation forms are completed, by the physician, for each medical appointment attended
  • Administer medications and ensures proper completion of necessary documentation
  • Coordinates all leisure and volunteer opportunities
  • Monitor the health and medical needs of individuals and immediately report any concerns
  • Drive individuals to medical appointments and activities in the community
  • Ensure program documentation and billable records are completed accurately and timely
  • Additional tasks and responsibilities may be assigned

Minimum Requirements:

  • High School Diploma or GED
  • Valid driver’s license with a satisfactory driving record
  • Minimum 90 days of relevant experience supporting individuals with intellectual and developmental disabilities
  • Ability to establish a comfortable and supportive relationship with individuals receiving supports
  • Ability to exercise good judgment and remain calm in crisis situations

Working Conditions:

  • Transport individuals utilizing your own vehicle or company provided vehicles
  • Schedule may change due to business needs and may include evening and weekend hours
  • May be required to be on-call in cooperation with other management staff
  • Frequent lifting, stretching, and other physical exertion may be required
  • May be required to lift or move 25+ pounds
  • May be exposed to various medical conditions and communicable diseases
