What You'll Do:

The Community Partnerships Coordinator facilitates partnerships that advance Adena Health's community health goals across its ni county service area. This role supports outreach, collaboration, and coordination with community partners while also incorporating a strong data focus. The Coordinator helps collect, analyze, and interpret community health data to inform decision-making, track progress against Community Health Needs Assessments (CHNAs) and Community Health Improvement Plans (CHIPs), and ensure accountability in reporting. 
Reporting to the Manager of Community Health, this position serves as an effective ambassador for Adena Health, strengthening relationships with local stakeholders and aligning community-based work with the health system's mission and strategic priorities. 

Minimum Qualifications 

Required Educational Degree: Bachelor's Degree 

Major/Area of Concentration:  

Preferred Education: Public Health, Public Relations, Social Work, or related field  

Required Certifications, Credentials and Licenses: De-escalation training within 6 months. 

Preferred Certifications, Credentials and Licenses: Community Health Worker (CHW) certification or equivalent 

Required Experience: 2+ years working with community partners to advance health initiatives 

Preferred Experience: Experience in healthcare or nonprofit settings
