Position Summary

The Community Habilitation Specialist provides hands-on assistance and training to individuals with developmental and intellectual disabilities who reside at home with their relatives. This role focuses on skill-building, promoting independence, and improving the quality of life for individuals through training, recreational activities, and goal-oriented programs.

Essential Job FunctionsSkill Development and Training
  1. Provide training in activities of daily living, including personal grooming, toileting, bathing, and hygiene practices.
  2. Train individuals in self-care tasks such as eating, dressing, and using grooming aids like hairbrushes, nail clippers, and razors.
  3. Teach household management skills, including meal preparation, shopping, laundry, and maintaining a clean home environment.
  4. Conduct travel training to promote independence in navigating public transportation and community resources.
  5. Provide health care training, such as scheduling medical appointments, practicing first aid, and maintaining nutritional habits.
  6. Support financial management skills, including money recognition, counting, and making purchases.
  7. Guide individuals in appropriate social behaviors at home and in the community.
  8. Enhance recreational skills and leisure interests through training and activities at home.
Social and Recreational Activities
  • Plan and implement community outings such as visits to parks, theaters, bowling alleys, and other recreational sites.
  • Provide opportunities for skill-building during community outings.
Respite Care and Goal Implementation
  • Offer respite care services for individuals living at home.
  • Implement individualized goals and skill programs per written staff action plans and methodologies.
  • Follow daily activity schedules to ensure structured program delivery.
Progress Monitoring and Documentation
  • Complete and maintain daily notes, documenting goal progress and recreational activities on required data sheets.
  • Monitor progress and implement the next steps in skill-building as outlined in the service program plan.
  • Prepare and submit monthly progress reports to the Community Habilitation Supervisor.
  • Communicate issues, concerns, and updates on participant progress with the supervisor regularly.

Qualifications
  • Minimum of a high school diploma or GED equivalent required.
