Job Title: Community Coordinator
Location: Hemet Hills Post Acute - Hemet, CA
Schedule: Full-Time, Field-Based (Self-Directed Weekly Schedule)
Pay: $18.00-$22.00 per hour, based on experience
Reports To: Administrator
Position Summary
Hemet Hills Post Acute is seeking a motivated and personable Community Coordinator to strengthen relationships that drive census growth and elevate our presence in the community. This is a field-based, entry-level opportunity ideal for someone looking to gain hands-on experience in healthcare operations, marketing, and admissions.
This role bridges community outreach, referral development, and basic marketing execution-ensuring consistent visibility of the facility while supporting the admissions pipeline.
Key Responsibilities
Community Outreach & Relationship Building
  • Maintain a consistent schedule of in-person visits to Assisted Living and Board & Care homes within the service area
  • Build and nurture referral relationships in partnership with the Discharge Planning and admissions teams
  • Represent Hemet Hills Post Acute professionally as the face of the facility in the community

Hospital Rounding & Follow-Up
  • Conduct routine in-person rounding at partner hospitals to maintain visibility and support patient transitions
  • Complete structured post-discharge follow-up calls to families and residents
  • Identify and communicate any concerns to the appropriate internal team members

Tours & Admissions Support
  • Generate and maintain a personal pipeline of prospective residents through outreach efforts
  • Track and report on key metrics, including tours scheduled, tours completed, and admissions conversions
  • Support facility tours and collaborate with the team during admissions conversations

Marketing & Reputation Management
  • Manage and maintain the facility's social media presence (primarily Facebook)
  • Capture content during facility events, community visits, and resident activities
  • Support online reputation efforts through platforms such as Birdeye
  • Encourage and help facilitate authentic reviews from residents and families
  • Assist in promoting community events, partnerships, and internal milestones
Qualifications
  • High school diploma or equivalent required
  • Strong interpersonal and communication skills
  • Comfortable initiating conversations and building new relationships
  • Organized and able to track outreach efforts, tours, and follow-ups consistently
  • Self-motivated with the ability to work independently with minimal supervision
  • Resilient and able to maintain momentum in a results-driven environment
  • Valid driver's license and reliable transportation required
  • Interest in healthcare, senior care, or a related field preferred
  • Prior marketing or sales experience is not required
