Job description

Outreach Community Coordinator (EHE Program)

Position Summary
This position supports the Ending the HIV Epidemic (EHE) initiative by conducting community outreach, engaging individuals living with or at risk for HIV, connecting clients to medical care and supportive services, and promoting long-term retention in care. This role builds partnerships with community organizations, represents the agency at outreach events, and tracks program outcomes to improve service delivery.

Key Responsibilities

  • Conduct in-person and virtual outreach to high-priority populations and individuals at risk of falling out of care.
  • Coordinate and participate in community outreach events, health fairs, presentations, and partner meetings.
  • Build referral relationships with community organizations, healthcare providers, and social service agencies.
  • Facilitate warm handoffs and connect clients to HIV medical care, behavioral health, housing, transportation, and other supportive services.
  • Assess client needs, develop linkage-to-care plans, and monitor client engagement and retention.
  • Maintain accurate documentation, referrals, and case notes in electronic systems.
  • Track outreach activities and analyze program data to identify trends, gaps, and opportunities for improvement.
  • Represent the agency and EHE initiative while promoting culturally responsive, trauma-informed services.

Qualifications

  • Associate's or Bachelor's degree in Public Health, Social Work, Human Services, or a related field, or equivalent experience.
  • 1–2 years of experience in outreach, case management, client navigation, HIV services, or community health preferred.
  • Knowledge of HIV prevention, linkage to care, and social determinants of health preferred.
  • Strong communication, public speaking, organization, and relationship-building skills.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Ability to travel locally (up to 60%) to events, meetings and client workshops.

Preferred

  • Bilingual English/Spanish.
  • Experience with Ryan White, EHE, or other HIV-funded programs.
  • Experience working with diverse and underserved communities.
