As a Case Worker at Horizon, you will…

  • Conduct intake screenings to assess behavioral health needs and determine appropriate referrals at various courts in the 8th district.
  • Connect individuals to mental health and substance use treatment services, including SPOA, FACT, and SOS teams.
  • Follow up with patients and providers to ensure engagement and address barriers to treatment.
  • Maintain a comprehensive guide of local behavioral health and social service resources.
  • Facilitate communication between providers, courts, and state agencies.
  • Educate participants on court processes and terminology.
  • Assist with access to basic needs such as shelter, food, and social services benefits.
  • Maintain accurate documentation and enter data into the OMH Diversion Center Navigator system.
  • Coordinate with OMH to address system challenges and develop innovative solutions.

  • Bachelor’s degree in social work, counseling, psychology, or a related behavioral health or human services field required.
  • One year of professional experience in mental health, criminal justice, substance use, or related human services required.
  • Strong knowledge of local behavioral health and criminal justice systems preferred.
  • Background in case management and care coordination is a plus.

Hours:

This position is full time, Monday – Friday from 9:00am - 5:00pm with the possibility of evenings. This schedule may vary depending on when appearances at court are needed.

Location:

This position requires regular travel between various court locations and Horizon facilities, while maintaining a designated home base at 77 Broadway St, Buffalo NY 14203.

Compensation 

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $19.50 - $24.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

*Please note: this is a grant funded position* 

This position entails eligibility for a sign-on bonus of up to $1,500 for your commitment to work within our Navigator Grant for at least one year. A change in scheduled hours or location of employment may impact eligibility of bonus.

Physical demands associated with this position include:   

  • Sitting for extended periods, alternating between standing and sitting
  • Mobility required with repetitive wrist, hand, and finger movements
  • Handling light duties, occasionally lifting objects up to 20 pounds
  • Local travel between courts and other Horizon locations
