Summary Statement
The Behavior Health Case Manager II position at DPC performs a fundamental role in the recovery mission and compliance with inspection and audit criteria. The Behavior Health Case Manager II will interview clients and family members to obtain detailed social history. They will connect and collaborate with providers to locate housing and vital community supports. Other responsibilities include admissions tasks such as social assessments, interventions, progress notes, developing and implementing Treatment Plan goals/interventions.\r\n
Essential Functions
Essential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. Since class specifications are descriptive and not restrictive, employees can complete job duties of a similar kind not specifically listed here.\r\nThis class is distinguished from a licensed Social Worker as employees do not perform the following duties: diagnose individuals for mental, emotional, behavioral, addictive, and/or developmental disorders and disabilities; perform related client assessment or evaluation; provide psychotherapeutic treatment; or develop and implement assessment-based treatment plans. An employee in this class may participate as a member of a multi-disciplinary team to implement behavioral health services or treatment plans in accordance with Delaware Code, Title 24.\r\n\r\n\r\n\r\n Interviews clients, family members or any other persons deemed necessary to obtain the most complete case history possible.\r\n\r\n\r\n Provides guidance and relevant information to the client's family to enable them to facilitate solutions and promote positive interactions among family members.\r\n\r\n\r\n Evaluates information derived from client interviews, case history, and other pertinent sources and refers to proper channels for review.\r\n\r\n\r\n Prepares narrative, statistical reports and court reports as necessary.\r\n\r\n\r\n Provides input as a member of a multi-disciplinary team in the assessment of client human service needs and treatment planning, enabling the most beneficial placement in foster care, day care, nursing home, drug/alcohol rehabilitation or other special care facility.\r\n\r\n\r\n Prepares necessary forms of application for services or benefits. Contacts appropriate social agencies and coordinates referrals.\r\n\r\n\r\n Appears at court proceedings as required testifying to the social and environmental issues affecting the client.\r\n\r\n\r\n Provides technical guidance to organizations and staff regarding findings related to client care or provides consultation to other caregiving agencies.\r\n\r\n\r\n Provides consultative services to state and private agencies; may participate in community projects.\r\n\r\n\r\n Advocates for clients and families in the protection of rights; obtains consent for treatment.\r\n\r\n\r\n Uses automated information systems and computer software to research, review, evaluate, and report on health/human service data.\r\n\r\n\r\n May lead paraprofessional, professional or administrative support staff by assigning, monitoring, and reviewing work and providing on-the-job training, guidance, and direction.\r\n\r\n\r\n Communicates and interacts effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.\r\n\r\n\r\n Performs other related duties as required.\r\n\r\n\r\n\r\n\r\n\r\n

JOB REQUIREMENTS for Behavioral Health Case Manager II\r\nApplicants must have education, training and/or experience demonstrating competence in each of the following areas:\r\n\r\n Possession of a Bachelor's degree or higher in Behavioral or Social Science or related field. \r\n Six months' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs. \r\n Six months' experiencein providing clinical treatment, counseling or determining eligibility for services or benefits as part of a treatment team. \r\n Six months' experiencein interpreting laws, rules, regulations, standards, policies, and procedures. \r\n Six months' experiencein narrative report writing. \r\n\r\n
