Job description

Shawnee Health is seeking a Clinical Case Manager for our Shawnee Health Williamson County locations. This position reports directly to the Clinical Case Manager Coordinator. The Clinical Case Manager will have a variety of responsibilities focusing on empowering Shawnee Health patients by reducing barriers to care, increasing patient participation in their own healthcare, and advocating on behalf of patients and their families.
The Clinical Case Manager works closely with Shawnee Health medical providers, case managers, community health workers, and counseling staff to reach patients and community members at risk of substance use disorders, including but not limited to opioid use, in Williamson County. The Clinical Case Manager works with primary care teams and other agencies to improve patient care outcomes, close care gaps, and assist the patients in navigating the health care and social service systems. Through education and appropriate linkage to community-based resources, at risk patients are able to better manage their own health.
Responsibilities
Assesses for social determinants related to health disparities
Connects patients to resources to improve overall physical and emotional health outcomes
Assists patients with completing applications for local, state, and federal benefit programs
Requirements
Bachelor’s degree in related field and additional two years Social Service related field
Must have excellent communication skills, both written and verbal in addition to interpersonal and organizational skills.
Excellent reading and writing skills.
Starting salary – $43,888-$47,958 with an increase upon successful completion of new hire probationary period. Compensation may be negotiable and is commensurate with experience and qualifications.
