They Specialize Delivering Evidence-Based Care Individuals With Chronic Behavioral Health Conditions, Including Histories Homelessness, Severe Mental Illness, Substance Disorders, Criminal Justice Involvement, Complex Medical Needs.
The Position :
Clinical Case Manager Provides Full Spectrum Clinical Case Management Services Within Scope Licensure. This Role Supports Individuals Experiencing Severe Mental Illness Co-Occurring Disorders Through Therapy, Service Planning, Coordinated Care.

Requirements :
  • Master's Degree Social Work, Mental Health Counseling, Marriage Family Therapy
  • Active Utah License (CMHC, LCSW, LMFT)
  • Prior Case Management Experience Required
