Job description

The LPN Transfer Nurse Coordinator is responsible for providing professional nursing care relative to room and bed assignment, coordinates room and bed assignments with nursing unites, nurse managers and nursing supervisor; coordinates with physicians and physician’s office staff. Assist Transfer Center Coordinator (TCC) in data gathering. Responsible for admissions in house transfer in STAR.
 
Education:  LPN Nursing Diploma required
 
Licensure:  LPN license required
 
Experience:  1-3 years preferred
