Job description

Care Manager Assistant Bilingual Mandarin or Cantonese

Salary Range: $35 - $45k

Location: Lower East Side, NY plus Travel

Essential Duties and Responsibilities:

The Care Manager Assistant is responsible for working closely with department staff to support them for smooth operation. The key responsibilities of this position include:

  • Assist the Care Manager in processing referrals for members.
  • Participate in IDT meetings with supervision from CMs supervisors.
  • Accompany CMs/CM supervisors to monthly home visits.
  • Collect CANS supporting documents for Yearly Eligibility Determination.
  • Scan, file, and organize forms and documents in drawers/shelves as directed.
  • Enter data into computer systems.
  • Call members/parents to complete satisfaction survey for Children's Health Home.
  • Complete new enrollment consent forms.
  • Prepare staff meeting documents.
  • Complete additional responsibilities or special projects assigned by your Supervisor.
  • Your schedule and responsibilities remain subject to change as determined by the Company and based on the needs of the Agency and the needs of the individual.
  • Promptly return phone calls and respond to emails (within 24 hours, or on the next business day), whether the inquiry is made by your Supervisor, other Hand in Hand staff, or is from a client, their family, legal guardian, a therapist, and other related individuals to your position.
  • Maintain full compliance with all company policies, procedures, and applicable regulations.

Education and/or Work Experience Requirements:

  • Bachelors degree required; preferred fields of study are Psychology, Sociology, Social Work, or Human Services.
  • Knowledge of the current code of ethics, standards and procedures outlined by the profession
  • Demonstrated excellent verbal and written communication skills, with the ability to effectively communicate with staff and families.
  • Demonstrated ability to communicate with members/parents in their home language.
  • Demonstrated proficiency in Microsoft Office (Word, Excel, Outlook)
  • Demonstrated ability to work effectively under pressure and meet deadlines, while maintaining a positive attitude and providing exceptional customer service
  • Demonstrated ability to work independently and complete assignments in accordance with given instructions, established routines, and standard practices

Physical Requirements:

  • Able to maintain punctual attendance with consistency.
  • Able to travel around New York City using public transportation.
  • Must be able to organize documents & folders in order.
  • Dress in professional attire to work Monday to Thursday. Can dress down on Fridays. Not allowed to wear jeans with holes or T-Shirts with big graphics.
  • Must be able to lift and carry up to 35 lbs.
