Administrator / Director of Operations - Home Health Services

Full Time

We’re seeking an experienced Home Health Administrator / Director of Operations who is passionate about clinical excellence, operational performance, and team development.

This is a senior leadership opportunity for a licensed Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) with proven experience in home health operations, Medicare compliance, and census growth.


Position Summary

The Administrator / Director of Operations – Home Health provides strategic and operational leadership for the home health agency. This role is responsible for clinical oversight, regulatory compliance, financial performance, staff leadership, and patient outcomes, while driving growth and maintaining the highest standards of care.


Key Responsibilities

·         Provide overall operational and clinical leadership for home health agency operations

·         Ensure compliance with Medicare Conditions of Participation, state regulations, and company policies

·         Oversee OASIS accuracy, PDGM and RCD compliance, and QAPI initiatives

·         Drive census growth through referral management and market engagement

·         Ensure high‑quality, patient‑centered care and interdisciplinary coordination

·         Lead budgeting, financial performance, and productivity management

·         Recruit, develop, and retain high‑performing clinical and operational staff

·         Maintain effective infection control, patient rights, and risk management programs

·         Utilize MatrixCare and performance data to drive outcomes


Required:

  • Active, unrestricted RN, PT, or OT license
  • Proven leadership experience of at least two (2) years in home health administration or operations
  • Strong knowledge of Medicare home health regulations
  • Demonstrated success in staff leadership, compliance, and census growth
  • Bachelor’s degree with training and experience in healthcare administration

Preferred

  • Previous experience as a Home Health Administrator, Director of Operations, or Clinical Leader
  • Experience managing multiple disciplines or multiple locations
  • Strong analytical, communication, and leadership skills
