The Program Coordinator maintains a continuum of professional and administrative responsibilities related to the coordination and tracking of assessment and evaluation referrals, including regular interaction with the community, families, and referral sources.

  • Attends mandatory staff meeting and other meetings as assigned
  • Receives and tracks incoming referrals
  • Requests and gathers pertinent information and documentation from referral sources
  • Collaborates with families and team members for efficient scheduling
  • Maintains evaluators schedule to ensure timely scheduling of appointments
  • Communicates with families and referral sources regarding the status of referrals
  • Manages data in the EHR system
  • Seeks the best use of materials, equipment, and staff to maximize efficiency and effectiveness
  • Acts as a good steward of Damar and/or client finances
  • Takes pride and ownership in Damar's facilities and assigned work areas
  • Conducts regular file audits to ensure alignment of billable hours, progress notes, invoices, and other relevant reporting documentation
  • Actively participating in and/or overseeing data collection and the use of data to improve outcomes, practices and service delivery
  • Maintains documentation that is accurate and legible
  • Completes and updates mandatory training and other licensure related items in a timely manner
  • Will adhere to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual
  • Ensures updated and thorough documentation is completed for each assigned independent contractor and client - Documentation is expected to be accurate and legible

What you need to be considered:

  • High School Diploma or GED required; Bachelor's degree preferred
  • Minimum of 2 years of experience in case management required
  • Minimum age of 18 per licensure requirements
  • Ability to pass and maintain CPR Certification
  • Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02
  • Pre-Employment and Employee Background Checks
  • Valid Indiana driver's license and driving record that meets eligibility requirements of Damar's insurance carrier (for all positions requiring driving)
  • Successful completion of pre-employment physical required
