Job description

245D Program Director / Designated Manager


We are seeking an experienced 245D Program Director / Designated Manager to lead the operation, compliance, development, and expansion of our Minnesota Home and Community-Based Services (HCBS) program.

This is a senior leadership role for someone with strong, hands-on knowledge of Minnesota 245D services who can take ownership of the program, build strong operational systems, and work directly with executive leadership to expand the organization.


Leadership & Operations

  • Lead day-to-day operations and overall performance of the 245D program.
  • Serve as the Designated Manager and oversee Designated Coordinator functions, as applicable.
  • Develop effective systems for admissions, service delivery, documentation, staffing, and program oversight.
  • Recruit, supervise, develop, and hold program staff accountable.
  • Establish strong operational standards as the organization grows.


Compliance & Quality

  • Maintain compliance with Minnesota Chapter 245D, DHS licensing requirements, and HCBS standards.
  • Maintain readiness for DHS licensing reviews, audits, investigations, and inspections.
  • Oversee required assessments, support plans, documentation, staff training, incidents, medication-related systems, and corrective actions.
  • Conduct internal compliance reviews and proactively address deficiencies and operational risks.
  • Monitor regulatory changes and ensure required changes are implemented.


Program Development & Expansion

  • Lead the development and expansion of 245D services, programs, and locations.
  • Evaluate new referrals and opportunities based on client needs, staffing capacity, compliance requirements, and organizational readiness.
  • Develop the operational structure necessary to successfully launch and sustain new programs.
  • Build strong relationships with counties, case managers, guardians, families, and community partners.
  • Work directly with executive leadership to identify expansion opportunities and execute the organization's long-term 245D strategy.


Qualifications

  • Significant experience working with Minnesota 245D licensed services.
  • Previous experience as a Designated Manager, Program Director, Designated Coordinator, or comparable 245D leadership position.
  • Strong knowledge of DHS licensing, 245D compliance, documentation, staffing, and service delivery requirements.
  • Experience with DHS licensing reviews, compliance audits, corrective actions, and program development strongly preferred.
  • Proven leadership, organizational, and problem-solving abilities.
  • Must meet applicable DHS qualification requirements.
