Case Manager
Ultimate Sustainability, “US”, an innovate program providing support for young adults with Autism and related conditions, is seeking an experienced Case Manager. The Case Manager will manage individual cases and add to the overall leadership of our team. If you have a passion for working with young adults and have experience working in a licensed 245D program, this is the role for you!
Responsibilities:
Oversee the program services Participants receive including daily visits, regular follow up with caregivers and case managers, and leading the Participant intake process·
Work closely with community members and government agencies during the development and execution of individual Participant services
Monitor Participant attendance and address absences directly with caregivers and case managers
Maintain compliant Participant files both hard copy and in Pavillio Cache
Ensure Participant annual reviews are completed
Coordinate, with the support of the Administrative Assistant, daily and weekly calendars
Ensure accurate billing procedures are followed, including submission for reimbursement and providing reconciliation reports to the team
Provide weekly updates to the management team, and maintain a daily communication flow with all team members
Support the onboarding and annual training of Direct Service Professionals to ensure Participants receive the highest level of service possible
Other duties as assigned
Required Qualifications:
Excellent communication skills and the ability to work collaboratively in a team to create exceptional value
Proficiency in EVS/ESS platorms
Proficiency in 245D compliance and credentialing
Strong problem-solving and project management skills
Demonstrated ability to be highly proactive and detail-oriented in a fast-paced environment
Bachelor's degree in social work or 3+ years of related experience preferred
·Valid Driver's license, vehicle registration and insurance
Must pass the NetStudy DHS background check
Knowledge of autism and autism and related conditions and related therapeutic techniques
