Key ResponsibilitiesProgram & Service Coordination
  • Coordinate and oversee services for individuals enrolled in NJ DDD programs.

  • Conduct intake assessments and complete all required intake documentation.

  • Develop and implement individualized service plans based on each person's needs, goals, and preferences.

  • Ensure participants receive quality, person-centered supports that promote independence and community inclusion.

Community-Based Services
  • Support individuals in accessing community activities, appointments, employment opportunities, and social engagement.

  • Coordinate referrals and connect individuals with appropriate resources and service providers.

  • Maintain strong relationships with families, support coordinators, community partners, and external agencies.

Leadership & Staff Supervision
  • Supervise and support direct care staff and program personnel.

  • Provide training, coaching, and performance feedback.

  • Foster a positive, collaborative, and accountable team culture.

Compliance & Administration
  • Ensure compliance with NJ DDD regulations, agency policies, and program requirements.

  • Maintain accurate records, case notes, reports, and service documentation.

  • Monitor service quality and identify opportunities for continuous improvement.

  • Prepare required reports and documentation for audits, funding sources, and agency leadership.

QualificationsRequired
  • Experience working with individuals with developmental disabilities.

  • Knowledge of New Jersey DDD regulations and community-based services.

  • Experience completing referrals, intake assessments, and service documentation.

  • Strong organizational, communication, and problem-solving skills.

  • Ability to supervise and support staff effectively.

  • Experience preparing reports and maintaining compliance-related documentation.

Preferred
  • Bachelor's Degree in Social Work, Human Services, Psychology, Behavioral Health, or a related field.

  • Previous management, coordination, or supervisory experience in developmental disability services.

  • Experience with NJ DDD electronic systems, including iRecord.

  • Knowledge of behavioral health systems and person-centered planning practices.
