As a DDD Coordinator, you will play a critical role in coordinating services, supervising staff, ensuring compliance with NJ DDD regulations, and building strong relationships with individuals, families, and community partners. Key Responsibilities

Program Coordination & Case Management

  • Coordinate and oversee community-based services for individuals with developmental disabilities.

  • Conduct intake assessments and complete all required intake documentation.

  • Develop, implement, and monitor individualized service plans based on each individual's goals, strengths, and support needs.

  • Coordinate referrals to appropriate community resources, programs, and service providers.

  • Support individuals in accessing community activities, appointments, employment opportunities, and other meaningful experiences.

Leadership & Staff Supervision

  • Provide supervision, guidance, training, and ongoing support to Direct Support Professionals and other program staff.

  • Conduct staff performance evaluations and ensure adherence to agency policies and procedures.

  • Foster a positive, collaborative, and person-centered team culture.

Compliance & Quality Assurance

  • Ensure compliance with all NJ DDD regulations, agency policies, and funding requirements.

  • Maintain accurate documentation, client records, progress notes, and required reports.

  • Monitor program quality and identify opportunities for continuous improvement.

  • Utilize the NJ DDD electronic platform/iRecord system to maintain service records and documentation.

Community & Family Engagement

  • Build strong partnerships with individuals served, families, guardians, support coordinators, and community organizations.

  • Serve as a resource and advocate to ensure individuals receive the services and supports they need to achieve their goals.

Qualifications

Required

  • Experience working with individuals with developmental disabilities.

  • Knowledge of New Jersey DDD regulations and community-based support services.

  • Experience coordinating services, completing referrals, and managing client documentation.

  • Strong organizational, communication, and problem-solving skills.

  • Ability to supervise and support staff effectively.

  • Experience completing reports and maintaining compliance documentation.

Preferred

  • Bachelor's degree in Social Work, Psychology, Human Services, Behavioral Health, or a related field.

  • Previous experience in program management, case management, or developmental disability services.

  • Familiarity with the iRecord system or NJ DDD electronic documentation platform.

  • Experience managing budgets, program resources, and quality assurance initiatives.
