About Us: ASG Home Care NJ LLC is a growing NJ DDD-approved home care agency providing Community Based Supports, Individual Supports, and Respite services to individuals with intellectual and developmental disabilities. We are a lean, fast-moving operation built on reliability, responsiveness, and genuine care for the people we serve.

About the Role: We are looking for an organized, detail-oriented Care Coordinator to join our team as we grow. This is a part-time role with real growth potential as the agency scales. You will be the operational backbone behind our active caseload -- making sure DSPs show up, service notes get submitted, authorizations are tracked, and nothing falls through the cracks.

This role is ideal for someone with DDD or home care experience who wants flexible hours and the opportunity to grow with an agency from the ground up.

What You Will Own:

  • Day-to-day coordination of active cases -- scheduling, shift coverage, and DSP communication

  • Service note compliance follow-up with DSPs

  • DSP onboarding tracking -- credentials, certifications, and required documentation

  • SDR and authorization tracking -- proactive monitoring of expiration dates

  • Referral intake support -- collecting required documents and coordinating case starts

  • Communication with families and Support Coordinators on active cases

  • Flagging compliance or staffing issues before they escalate

What We Are Looking For:

  • Experience in a DDD, home care, or IDD services environment strongly preferred

  • Familiarity with NJ DDD programs, ISPs, SDRs, and EVV compliance is a major plus

  • Highly organized with strong follow-through -- this role lives and dies on details

  • Comfortable communicating directly with DSPs, families, and Support Coordinators

  • Able to work independently without constant direction

  • Experience with HHAeXchange is a plus but not required
